Job Summary:

Establish and maintain agency contacts with area health facilities and community organizations. Facilitate client transfer between the facilities and the hospice home care and inpatient unit to ensure continuity of care. Provide education to the community on the Hospice Philosophy of Care. Demonstrate respect, dignity, kindness, and empathy in each encounter with all patients, families, visitors, and other employees regardless of cultural background.

Job Description:

Responsibilities

  • Develop and maintain regular contact with area physicians and health care facility discharge planners, social workers, and health care coordinators to maintain the flow of communication.
  • Update health care professionals on hospice guidelines and policies. 
  • Conduct pre-discharge hospital visits at the physicians, patient/familys request, assessing clients for hospice program eligibility.
  • Assist in maintaining standardized operational processes in compliance with St. Elizabeth Healthcare and hospice policies, legal requirements, and regulatory mandates. 
  • Provide back-up support and handle additional functional role responsibilities, as required, to assure operational needs are met. 

Communication

  • Apprises the Admissions Coordinator, Leadership or Multidisciplinary Team of referral to facilitate admission of the client. 
  • Coordinate feedback to referral source regarding acceptance and progress of client, as necessary. 
  • Facilitate communication with facility when St. Elizabeth Hospice clients are discharged to a facility to enhance coordination of services.
  • Responsible for maintaining a positive relationship with referral sources and community organizations concerned with hospice. 
  • Facilitate positive communication between St. Elizabeth Hospice and the client population.

Documentation

  • Meet all documentation timeframes and requirements according to agency policies and guidelines. 
  • Follow all regulatory requirements in providing patient care and documentation of services.                      

CQI/Peer Review

  • Participates in Performance Improvement and chart reviews.

Education

  • Provide health education and community resource information to patients and families. 
  • Maintain role as a patient health advocate while the patient is on service. 
  • Meet all skills and competencies identified by the hospice department and healthcare system.
  • Perform other duties as assigned.

Education, Credentials, Licenses:

  • Graduate of an accredited school of nursing; licensed to practice nursing in the state where work is being performed. 
  • Meets contact hour requirements for licensure, including all state-required courses.
  • Valid Drivers license.

Specialized Knowledge:  Demonstrates advanced assessment skills appropriate for home care environment, ability to function independently; knowledge of community resources.

Kind and Length of Experience: 

2-3 years of RN experience in a hospital or care facility setting.
