Job Summary:

Responsible for program and project management; ensure timely and effective outcomes. In collaboration with Care Coordination Services Management, is responsible for the oversight, management, implementation, and enhancement of process changes within the Care Coordination Division and in collaboration with interacting departments if applicable.

Job Description:

Provide oversight for the management, implementation, and enhancement of new or changing processes within the Care Coordination Division

  • Serve as the project lead for Nursing/Social Work for enhancements and technology implementations.
  • Attend Nursing Leadership, and Nursing Professional Development Council to stay abreast to changes occurring throughout the system

Lead process improvement efforts related to clinical and EPIC documentation standards as requested.

  • Act as project/committee lead or active participant in clinical work groups aimed at the improvement of clinical activities and standardization of EPIC documentation. 
  •  Uses effective change management practices to ensure positive outcomes.
  • Lead or participate in process improvement efforts to support goals/outcomes of special clinical workgroups.
  • Education team

Directs and coordinates the review and revision of policies and procedures.

  • Lead or contribute to projects/committees that establish guidelines, set procedures, outline standards, and monitor for compliance.
  • Ensure the timely review and revision of clinical policies and procedures.
  • Ensure system-wide consistency related to clinical policies and procedures.
  • Collaborate with staff development to ensure education of the staff related to policy and procedure changes

Collaborate with the Patient Safety and Accreditation team to enhance awareness and ensure compliance with documentation.

  • Assist with Failure Mode and Effects Analysis and Root Cause Analysis processes.
  • Maintain knowledge of applicable standards
  • Serve as member of the Patient Safety and Accreditation meeting.
  • Assist with compliance monitoring within Care Coordination and assist with Joint Commission preparation

Business Relationships/Patient Experience

  • Establish and maintain relationships across the organization to support safe and effective patient care.
  • Maintain knowledge of organizational policies and procedures
  • Maintain knowledge of operational workflows
  • Safety/Patient Experience rounding to be performed in all Care Coordination dept.

Serve as Support for Care Coordination Outreach Programs

  • Collaborate with Care Coordination manager in Community Programs hosted by Care Coordination Services (i.e., students, etc.)
  • Participate in Community Partner Expos
  • Performs other duties as assigned.

Education, Credentials, Licenses:

  • BSN, Graduate of accredited School of Nursing. Licensed to practice nursing in the state where work is being performed. Meets contact hours requirement for licensure, including all state-required courses.

Specialized Knowledge: 

  • Demonstrated strong project management skills.
  • Strong communication and follow-up skills.
  • Ability to direct self and others and interpret policies, procedures, and standards.
  • Ability to lead meetings and training classes.
  • Communicate and work in a highly collaborative manner.
  • Knowledge of change management principles.
  • 5 years clinical experience with Care Coordination setting

Kind and Length of Experience: 

  • Previous experience in leadership roles such as Manager, Nursing Supervisor, Clinical Educator or Assistant Nurse Manager
