Job Summary:

Provides psychosocial assessments, assists in care planning in coordination with the members of the Palliative Care team, Hospital Staff and Physicians. Provides counseling services to patients/families.

Job Description:

ASSESSMENT AND COUNSELING:

  • Provides psychosocial assessments of patient and family.  Initiates patient/marital/family counseling related to end of life issues including grief, loss, life integration and caregiver stress, enhancing patient/family problem solving and coping abilities. 

COMMUNICATION:

  • Facilitates patient/ family meetings based on needs. Collaborates with Palliative Care team members, hospital staff, patients and families to develop goals and plan of care.

EDUCATION:

  • Educates patients and families on Advance Directives such as Living Wills and DNR documents. Promotes the Palliative Care program by educating physicians, the public, healthcare personnel and organization in the community about palliative care.   

RESOURCE IDENTIFICATION:

  • Identifies hospital and community based services which will meet patient/family needs and provides referrals and advocacy in obtaining services. 
  • Assists patient/families with financial and insurance issues regarding Social Security Disability and Medicaid applications as needed.      

PERSONAL GROWTH AND DEVELOPMENT:

  • Maintains skills and knowledge as appropriate to palliative care and role.
  • Meets continuing requirements for the Board of Social Work in the state where work is being performed.

PERFORMANCE IMPROVEMENT:

  • Assists in the process to evaluate patient outcomes, financial and clinical results that can be tracked to measure the success of the Palliative Care Program.

LEADERSHIP:

  • Provides leadership and support in the promotion of palliative care services including research and fund development, marketing and public relation strategies.
  • Performs other duties as assigned.

Education, Credentials, Licenses:

  • MSW from accredited school by the Council on Social Work Education. Licensed Clinical Social Worker or Certified Social Worker willing to participate in active supervision toward achieving LCSW.

Specialized Knowledge: 

  • Knowledge of Palliative Care and the psychosocial issues of terminally ill patients and their families. Excellent communication skills and the ability to coordinate care well with an interdisciplinary team. Knowledge of Emotional and Mental Disorders and the DSM V diagnoses applicable to palliative care.

Kind and Length of Experience: 

  • 2 years social work experience in a medical setting.
