Provides clinical social work and discharge planning services to patients and their families in collaboration with the interdisciplinary team and post-acute care providers.Demonstrate respect, dignity, kindness and empathy in each encounter with all patients, families, visitors and other employees regardless of cultural background.
Job Description:
Assessment and Evaluation
- Conducts a psychiatric psychosocial assessment and evaluation of patients regarding various emotional and mental disorders, using the DSM V.
Clinical Interventions
- Creates a positive therapeutic rapport with patients while providing individual mental health counseling and crisis intervention.
Resource Identification
- Identifies hospital and community based services which will meet patient/family needs and provides referrals and advocacy in obtaining services.
Patient/Family Education
- Provides psycho-education to patients and families to empower patient and enhance problem solving and coping skills.
Other Social Service Needs
- Assesses and reports child and adult abuse/neglect;
- Provides counseling and referrals re: chemical dependency;
- Offers grief counseling to families related to death/dying or to accepting chronic and terminal illnesses;
- Assists with discharge planning activities for those patients being discharged from the hospital.
Personal Growth and Development
- Maintains skills and knowledge as appropriate to the medical social work role.
- Participates in continuing education to fulfill licensure requirements.
Performance Improvement
- Assists in the process to evaluate patient outcomes, financial and clinical results that can be tracked to measure success of the Social Service Department.
Performs other duties as assigned.
Education, Credentials, Licenses:
- Graduate of an accredited masters school of social work education.
- Licensed as Masters prepared social worker in state where work is being performed.
Specialized Knowledge:
- Knowledge of emotional and mental disorders and mental disorders and the DSM.
- Excellent communication skills and the ability to coordinate care well with an interdisciplinary team.
