Job Title: Ambulatory Care Pharmacist- Pharm.D.

PRIMARY PURPOSE:

Reports to the Director of Value Based Performance, the Ambulatory Care Pharmacist is part of a team based collaboration that is primarily responsible for developing and implementing pharmacy related processes and procedures within St. Elizabeth Physicians to improve organizational performance on selected value based contracts driven by quality, utilization, and efficiency metrics.

DUTIES AND RESPONSIBILITES:

  • Identification of medication-related gaps in care opportunities for population health financial incentives.  
  •  Maintains an open line of communication with the Director of Value Based Performance and other team members.
  •  Routinely monitors both internal and external pharmacy related reporting for opportunities for improvement.
  • Collaborates with both employed and non-employed pharmacy staff.
  • Develops and maintains communications and collaborations between payers, practices and patients.
  • Develop and monitor protocols for closure of medication related population health care gaps.
  • Focus centered around but not limited to utilization metrics.  Including, but not limited to: ED Utilization, Hospital Admissions (Re-admissions), Medication utilization/reconciliation, and LOS.
  • Develops a provision of accountability at the practice level for optimization of contractual medication-related metrics
  •  Develops processes for provider education and communication.
  •  Develops action plans with physician leadership at both organizational and practice level.
  • Development and provision of a Medication Therapy Management (MTM) program which includes Comprehensive Medication Reviews (CMR) and Targeted Medication Review (TMR)
  • Direct collaboration with hospital based, community and payer employed pharmacists.
  • Evaluates organizational, practice and individual provider performance to organizational goals, payer contract goals as well as governmental initiative goals.
  • Educates organization and system on Value Based Initiatives as they relate to Pharmacy needs.
  • Oversees Ambulatory Pharmacy resources in their efforts to further Value Based performance, including utilization initiatives and Complex Case review program.
  • Collaborates with Utilization Lead on organizational, division, practice or individual provider action plans.  Communicate with providers on their data, including education on the data and action plans for improvement.
  •  Work with Utilization Lead in identifying case opportunities and communicating with the appropriate providers on action planning for improvement.
  •  Collaborates with other support and operational departments, including SEPN and HSN, regarding utilization action plans as necessary.
  • Works with analysts to facilitate reporting and benchmarks.
  • Works with MSO to initiate discussion and solve appropriate coding and revenue cycle issues.
  • Works with offices to facilitate PCMH initiatives and include with other organizational objectives.
  • Works with finance as needed on managed care initiatives.
  • Works with other Clinical Transformation colleagues on the development of new processes or procedures that meet the strategic goals of the organization.
  • Collaborates with St. Elizabeth Healthcare on system projects to meet system and organizational goals.
  •  Works with associates of SEPN/HSN on pharmacy related contracting measures.
  • Promptly and courteously handles all calls and interactions with internal and external collaborators.
  • Completely and accurately takes and distributes information.
  • Must be very organized and able to prioritize, coordinate and assist with the multiple projects.
  • Maintains current filing system for various utilization initiatives and outside projects. 
  • Meets deadlines as indicated. 
  • Works effectively with other professional groups and individuals
  • Other duties and responsibilities as assigned are complete on a timely, thorough and accurate basis.

REQUIRED SKILLS AND KNOWLEDGE:

Ability to manage and prioritize multiple tasks, knowledge of Excel, Word, Visio, Outlook and PowerPoint and the ability to learn other computer skills. Must have good organizational skills and work professionally with doctors, hospital administration and management, SEP employees and the public. 

OTHER REQUIRED SKILLS AND KNOWLEDGE:

  • Knowledge of various healthcare plans and modeling
  • Extensive knowledge of a variety of vendor software programs specific to quality and cost management 
  • Managerial experience required.
  •  Experience in developing improvement plans and measurable goals.
  •  Experience in directing and assisting others in stressful situations in a calm and confident manner.
  • Demonstrated ability to work successfully in a team-based decision-making culture.
  •  Demonstrated ability to work independently resulting in effective outcomes and on-time performance.
  • Experience in planning and coordinating multi-disciplinary communications strategies, strategic initiatives, and events.

EDUCATION:

Minimum: Doctor of Pharmacy (Pharm.D.)

LICENSES AND CERTIFICATIONS:

Minimum:  License to practice pharmacy in the state of Kentucky and to obtain an Indiana License before start date.

Valid Drivers License.

YEARS OF EXPERIENCE:

3+ years of pharmacy experience in an ambulatory setting, healthcare system, or insurance company preferred.  Previous collaboration with physician group desirable.
