Reports to Operations Manager, the Intake Specialist is primarily responsible for providing patient care services through social work intervention. They will help in population health by scheduling the patient with the appropriate Behavioral Health provider and provide resources for patients to improve problem-solving for social issues related to their health. Through patient intervention, compliance with treatment regimens, following up with appropriate teams and providers, and better utilization of services in the ambulatory healthcare setting will be achieved. The Intake Specialist works closely with the entire care team to provide optimal services to the patient. The Intake Specialist is responsible for creating a positive impression with patients, physicians and other visitors he/she encounters, both in-person and telephonically.Job Description:

Hourly rate starting at: $15.00
DUTIES & RESPONSIBILITIES:
1. Comply with all applicable laws and regulations.
2. Provides patient assessment to ensure safety and proper provider placement.
3. Coordinates and optimizes activities to develop high risk patient outcomes including:
a. Completes thorough chart review upon referral
b. Shares information with assigned provider. Office related to social needs and treatment issues of patient.
c. Participates in ongoing communication with physician and interdisciplinary team to develop a collaborative relationship aimed at improving clinical treatment goals.
4. Patient Education.
a. Educates patients through providing resources, timelines, and goals for improving their accountability and collaboration in taking part in their health program and medical plan.
b. Proactively makes appropriate referrals.
5. Provides strategies for frequently admitted patients and those who frequently utilize the ED.
6. Respond to referrals for services within 3-5 business days.
7. Respond to voice messages within 1 business day.
8. Completes required documentation on all patient encounters and updates in the EMR.
9. Identifies and documents all Intake intervention.
10. Communicates only appropriate necessary information on chart applicable to the referral source in accordance with HIPAA guidelines.
11. Participates in peer reviews, EMR reviews, employee education, clinical guideline development and other continuing education and quality assurance activities. Demonstrates compliance with standards, regulations, policies and procedures. Complies with patient confidentiality requirements and promotes patient advocacy.
12. Other duties and responsibilities as assigned are completed on a timely, thorough, and accurate basis.
13. Provides insight and guidance to other staff members as it relates to further needs of the patients.
REQUIRED SKILLS AND KNOWLEDGE:
1. Expert knowledge of community, professional and education resources.
2. Knowledge of basic nutrition, behavioral factors, and psychosocial/family systems.
3. Knowledge of common safety hazards and precautions to establish/maintain a safe work environment.
4. Knowledge of health promotions, health risk identification, and patient education principles and techniques.
5. Ability to utilize computers in documentation of activities and accessing of information.
6. Ability to utilize computer software programs such as Microsoft Word and Excel.
7. Demonstrated ability to work successfully in a team-based, decision-making culture.
8. Demonstrated ability to work independently resulting in effective outcomes and on-time performance.
9. Experience in planning and coordinating multi-disciplinary communications strategies, strategic initiatives, and events.
10. Must respond and follow through to requests from customers promptly.

12. Must utilize resources wisely.
13. Performs duties willingly and with initiative. Shares necessary information so co-workers can do the same.

OTHER REQUIRED SKILLS AND KNOWLEDGE:
1. Skill in oral and written communication including interviewing to gather social histories, providing presentations, consultations, facilitation, and documentation.
2. Knowledge of electronic medical records (EMR).
3. Skill in case management, time-management, problem-solving, crisis intervention, multitasking and other organizational matters.
4. Ability to analyze complex data and draw conclusions.
5. Ability of interpret, adapt, and apply guidelines and procedures.
6. Ability to establish/maintain effective working relationships with patients, clinical employees and the public.

EDUCATION:
High school Diploma/GED.
YEARS OF EXPERIENCE:
2 years of experience in community or non-community setting.
