Job Summary:

Reports to Practice Manager, the Diabetes Educator Dietician is responsible for providing education and/or resources on diabetes self-care management to patients/clients of varying levels of education living with diabetes throughout their life span. They participate as a member of the treatment team to support physicians both within the center and external physicians caring for patients with diabetes and other medical nutrition needs.

DUTIES AND RESPONSIBILITIES:
1.    Comply with all applicable laws and regulations.
2.    Provides medical nutritional therapy and education to patients/clients to support diabetes self-management and management of other endocrine disorders.
3.    Collaborates with physicians and other members of the diabetes treatment team to determine nutritional education needs and recommendations for patients/clients.
4.    Develops an individualized educational nutrition plan and recommendations for patients/clients.
5.    Implements and maintains the American Diabetes Association (ADA) approved curriculum with associated educational materials designed to meet patient/client needs.  
6.    Formulates and implements individualized educational plans which identify goals that include lifestyle counseling and goal achievement.  Ensure complete and accurate information in patient chart.
7.    Functions as a resource/mentor and collaborates with multidisciplinary healthcare team members with providing education.
8.    Provides effective communication, collaboration of activities within assigned specialty.
9.    Responsible for ensuring availability of specialty equipment and supplies for diabetes educations.
10.  OSHA, CLIA and HIPAA compliance.
11.  Assist with completion of patient requests.
12.  Timely and accurate filing/distribution of all patient information.
13.  Maintains necessary CEU requirement for licensure/ certification.
14.  Other duties as assigned.


REQUIRED SKILLS AND KNOWLEDGE:
1.    Ability to manage and prioritize multiple tasks, knowledge of Excel, Word, Outlook and PowerPoint and the ability to learn other computer skills. 
2.    Must have good organizational skills and work professionally with doctors, hospital administration and management, SEP employees and the public.   
3.    Demonstrated ability to work successfully in a team-based decision-making culture.
4.    Demonstrated ability to work independently resulting in effective outcomes and on-time performance.
5.    Experience in planning and coordinating multi-disciplinary communications strategies, strategic initiatives, and events. 
6.    Must respond and follow through to requests from customers promptly.
7.    Must work carefully and precisely with attention to detail.
8.    Must utilize resources wisely.
9.    Performs duties willingly and with initiative.  Shares necessary information so co-workers can do the same.  
10.  Cooperates with other departments and work groups.

LICENSES AND CERTIFICATIONS:
CDCES Certification (Certified Diabetes Care and Education Specialist) previously CDE (Certified Diabetes Educator).


EDUCATION: 
Minimum:  Graduate of accredited nutrition and dietetics program.  Current registration to practice dietetics in the state of Kentucky.  Certification as a Diabetes Care and Education Specialist.  Knowledge of educational principles and instructional media/computer skills.

YEARS OF EXPERIENCE:
Three years experience in clinical dietetics.
