- Reviews clinical information and completes new consultations and follow ups in a timely manner working with the interdisciplinary team (physicians, social workers, and chaplains)
- Act as an integral part of the treatment team determining goals of care for patients with cancer-related or life-limiting chronic illnesses; collaborate with palliative care physicians regarding symptom management needs.
- Provides extensive education regarding chronic diseases and how they impact quality of life.
- Communicates with treatment team including other providers on changes to plan of care.
- On call coverage required for palliative care
