Reports to the Social Work Manager, the Masters Level Externship Student Social Worker is a member of the Care Coordination Integrative Care team and located in throughout the primary care offices as a float to provide integrative mental health care services to patients in the care practices, in addition to assisting patients with their social determinate needs. The student is primarily responsible for face to face comprehensive biopsychosocial assessments and collaborate with the patients primary care physician and other medical staff to determine diagnosis and treatment through brief therapy and strive for a healthy mental health outcome.The student will also be providing possible face to face encounters with patients when necessary in various locations within the St Elizabeth service area such as in the primary care office, Medicaid office, or other community centers to assist in helping patients with social determinate needs. This position not only assists with counseling but will also help patients with resources (food/housing/homeless/nursing home placement/WIC/SSI/SSDI/Medicaid application, food stamps, Medicare, safety needs, DME, Medication assistance, etc). This role provides direct communication with physicians, nurses, health-care advocates, care coordinators; DME companies; community partners and agencies; lawyers; housing entities, etc.
The Extern will work closely with the entire integrative care team and primary provider to provide optimal services to the patient. The Extern demonstrates respect, dignity, kindness and empathy in each encounter with all patients, families, visitors and other employees regardless of cultural background.

EDUCATION:
Minimum: Enrolled in a Social Work Masters Program within last 2-3 semesters
YEARS OF EXPERIENCE:
Minimum: 0-2+ years of social work experience in mental health; prefer medical social work experience.

DUTIES AND RESPONSIBILITIES:
- Clinical Social Work: 1. Provides social work as a requirement of college curriculum under supervision, which focuses on the assessment and collaboration with patients primary care physician to determine diagnosis and treatment of an emotional and mental illness as related to the total health of the patient.
- Psychosocial Assessments & Communication: 2. The practice of clinical social work shall be based on knowledge of DSM-5 diagnosis, psychodynamics, human relations, crisis intervention and psychopathology.
- 3. Facilitates comprehensive psychosocial assessments, which examine the current problem(s), as well as relevant bio-psychosocial and environmental factors in order to establish an appropriate care plan. b. Communicates appropriate and applicable information on chart in accordance with HIPPA guidelines. c. Clinical work will be performed under the LCSW Board supervision. Supervision will be provided 1:1 weekly. d. Initiate contact with patient/family in the Physicians office or by phone within 1 business day from referral date. e. Creates a positive therapeutic relationship with the patient that leads to correction of the dysfunction. f. Provide a wide range of treatment modalities based on assessment which may consist of: short-term CBT, solution-focused, motivational interviewing; problem solving; crisis intervention; mindfulness; trauma[1]informed; behavior activation; acceptance and commitment, person-centered or any number of effective evidence-based options to stabilize and regulate the current dysfunction. g. Identifies social determinants that impair/impact the patients inability to pursue treatment or barriers that inhibit patients on-going health issues and works closely with the Outreach Specialist within the Care Coordination department to refer patients for social determinate needs. h. Treatment planning that includes development, implementation, and modification of the plan. i. Evaluation of progress on patients care plans. j. Termination of the treatment process or referral to higher level of care when necessary.
- Resource Identification: 4. Initiates contact with primary care patients via phone with primary care patients who have been discharged from the St. Elizabeth emergency room after a mental health evaluation for further mental health needs for therapy. Documentation: a. Documents all visits and phone contacts accurately and in a timely manner. b. identifies and documents all interventions and case closure plans in EPIC. c. Documentation to be reviewed by LCSW Supervisor.
- Patient/Family Education and Counseling: a. Initiates patient/family education, short term counseling and other social work interventions to enhance problem solving and coping. Acts as a liaison between patient/family and all members of the healthcare team. c. Continuously seeks to increase the quality and value of services provided to our patients/family and health care teams Builds rapport with patients/family and healthcare teams. d. Expands and/or strengthens patients social supports. e. Facilitates support groups as needed to support patient population. f. Provides yearly education to Primary Care staff on Suicide assessment. Team Consultation & Supervision: g. Serves as consultant to the interdisciplinary team regarding psychosocial components, procedures, or services to promote greater understanding of the patient. h. Communicates effectively with co-workers and healthcare team to ensure continuity of care and service. i. Participates in peer reviews, EMR reviews, employee, education, clinical guideline development and other continuing education and quality assurance activities, demonstrate compliance with standards, regulations, policies, and procedures. Complies with patient confidentiality requirements and promotes patient advocacy. j. Clinical work will be performed under the LCSW supervisor for 1:1 weekly. Employee Care: k. Behavior Standards: 1.
