The best career is right here.St. Elizabeth Healthcare is hiring for a Certified Social Worker position on the Hospice unit located at the Edgewood, Kentucky location.
Shift & Hours: 1st shift; Full-time, 32 hours weekly.

Provides Social Work services, which include psychosocial assessments, counseling, crisis intervention and other therapeutic interventions to hospice patients and their families. Serves as a member of the interdisciplinary team and coordinates patient care with the Case Manager. Demonstrate respect, dignity, kindness and empathy in each encounter with all patients, families, visitors and other employees regardless of cultural background.
Education, Credentials, Licenses:
MSW from school accredited by the Council on Social Work Education. Certified Social Worker (CSW).
Specialized Knowledge:
Knowledge of psychosocial issues re: health related problems and post-acute care needs. Excellent communication skills and the ability to coordinate care planning with an interdisciplinary team.
Kind and Length of Experience:
One year paid social work experience in hospice, medical or clinical setting, post MSW.
