Provides Social Work services, which include psychosocial assessments, counseling, crisis intervention and other therapeutic interventions to hospice patients and their families. Serves as a member of the interdisciplinary team and coordinates patient care with the Case Manager.Demonstrate respect, dignity, kindness and empathy in each encounter with all patients, families, visitors and other employees regardless of cultural background.
Job Description:
Pt./family counseling and education:
Provides crisis intervention, anticipatory grief counseling, family counseling and other clinical interventions to enhance problem solving and coping. Educates on end of life issues and provides appropriate linkages to community resources as needed.
Psychosocial Assessment:
Examines Pt/familys current functioning as well as bio-psychosocial and environmental factors to develop an appropriate care plan for hospice patients.
Communication:
Regularly attends and actively participates in interdisciplinary patient care conferences and collaborates with members of the Interdisciplinary team (IDG) regarding the plan of care. Serves as a consultant to the IDG on psychosocial issues to increase understanding of patient/family needs. Assists the staff in maintaining healthy professional boundaries and attending to self-care to remain effective at work.
Documentation:
Documents information regarding patient/family status, care plans and clinical interventions on appropriate forms.
Personal Growth and development:
Maintains skills and knowledge of hospice social work. Seeks out opportunities to enhance knowledge and clinical effectiveness. Meets continuing requirements of licensure for the Board of Social Work in the state where work is being performed.
Performance improvement:
Helps to identify areas for improvement in hospice patient care, collects data as needed and evaluates the effect on patient satisfaction.
Performs other duties as assigned.
Education, Credentials, Licenses:
MSW from school accredited by the Council on Social Work Education. Certified Social Worker (CSW)
Specialized Knowledge:
Knowledge of psychosocial issues re: health related problems and post-acute care needs. Excellent communication skills and the ability to coordinate care planning with an interdisciplinary team.
Kind and Length of Experience:
One year paid social work experience in hospice, medical or clinical setting, post MSW.
