Job Summary:

St. Elizabeth Healthcare is hiring a Registered Nurse Program Coordinator for our Heart and Vascular Institute located within the Edgewood Hospital.

Shift/Hours: Full-Time – 40 hours/week, First Shift

The RN Program Coordinator oversees the development, implementation, and provide maintenance for all aspects of the identified HVI specialty program including navigation of patients in collaboration with the multidisciplinary team. To coordinate and manage the disease specific program and patient care of this population throughout the continuum to optimize the patient’s clinical, functional and psychosocial status. Provides coordination of the program and care among the multidisciplinary health care team including community outreach. Provides oversight to disease specific certification activities if applicable.

Job Duties:

  • Manage the HVI specialty program and overall care of the patient population across the continuum (acute care inpatient, ambulatory and post-acute care) in collaboration with the HVI providers and other members of the multidisciplinary team.

  • This involves program development, advanced assessment, planning and coordination of care, modification of the treatment plan as indicated and monitoring and evaluation of outcomes.

  • Oversee and provide clinical consultation and education related to the management and care of HVI specialty program patients for staff, physicians as well as other disciplines who may have contact or care responsibilities for this population. This includes community outreach to raise awareness of the program.

  • In collaboration with the Program Medical Directors and Department leadership, oversees the HVI specialty program including performance improvement and certification (as applicable) to improve patient outcomes.

  • Navigates the patient and caregivers across the care continuum in collaboration with the HVI RN Navigators; maximizes the patient’s participation and control in their own recovery

  • Facilitates the access and utilization of community resources and organizations to assist in the patient’s recovery.                                     

  • Demonstrates leadership skills such as time management, coordinating and meeting multiple patient and family needs and requests, meeting physician and team member expectations, setting priorities and networking.

  • Demonstrates commitment to professional growth and competence.

  • Performs other duties as assigned.

Qualifications:

Education, Credentials, Licenses:

Registered Nurse in state where work is being performed

If eligible, must have a Compact (multi-state) License in their primary state of residence.

Bachelor’s degree in nursing

BLS (Basic Life Support) certified

Specialized Knowledge: 

Strong interpersonal, communication and education skills

Extensive knowledge of nursing standards and practice related to cardiovascular disease

Kind and Length of Experience: 

5 years nursing experience (at least two years of program specific experience)

DESIRABLE

Education, Credentials, Licenses:

Master’s degree in nursing

Specialty certification

Kind and Length of Experience: 

Experience working with patient population

Previous program coordinator experience

3+ years of experience in a similar role
