Job Summary:

The RN Case Manager is responsible for provision of quality care to all patients and the achievement of excellent patient outcomes. The RN Case Manager assesses patient needs, assists in the formulation and implements the physicians plan of care to reach achievable goals mutually agreeable to the patient and care providers. These activities are carried out in a collaborative manner that promotes quality, continuity and safety.

Job Description:

Responsibilities:

  • Assumes full responsibility for case management of a specific caseload of patients. 
  • Work collaboratively with the patient/family/caregiver and multi-disciplinary team to coordinate services to meet the health needs of the patient as identified in the plan of care. 
  • Reevaluate the plan of care and reassess patient as necessary and in accordance with all federal, state, and regulatory guidelines. Manage patient care and goal realization within established standard and resource utilization guidelines.
  • Use the Nursing Process in providing care for patients.
  • Make decisions independently and within scope of practice regarding patient care and needs during visit.   
  • Has the ability to adjust to the unique situations encountered in the patients home.  
  • Adhere to all policies and procedures applicable to the hospice program and St. Elizabeth Healthcare. 
  • Participate in on-call rotations as assigned.

Documentation:

  • Documents within the EMR or downtime forms pertinent information regarding patients condition.
  • Completes documentation to support visits and completes discharge and transfer documentation.
  • Meets timeframes required for documentation submission. 
  • Assures documentation is accurate, complete, specific and meets all state, federal and JCAHO regulations.

Communication:

  • Communicate with all members of the multidisciplinary team to coordinate care and services. 
  • Keep multidisciplinary team informed of pertinent changes in patient status to promote smooth flow of information through the department. 
  • Work with the team to provide efficient and effective care that meets the patients defined goals.

Education:

  • Provide health education and community resource information to patients and families. 
  • Maintain role as a patient health advocate while the patient is on service. 
  • Meet all skills and competencies identified by the hospice department and healthcare system.

CQI/Peer Review:  

  • Participates in Performance Improvement and chart reviews.
  • Performs other duties as assigned.

Education, Credentials, Licenses:

  • Graduate of an accredited school of nursing; licensed to practice nursing in the state where work is being performed. 
  • Meets contact hour requirements for licensure, including all state-required courses.
  • Valid Drivers license.

Specialized Knowledge: 

Demonstrates advanced assessment skills appropriate for home care environment, ability to function independently; knowledge of community resources.

Kind and Length of Experience: 

1 year of RN experience in a hospital or care facility setting.
