Job Summary:

Facilitate the timely transition of patients into the department programs. Works closely with the referral sources to provide for collection of necessary information to start patient care. Collect data, pre-authorization and authorizations for services provided by the agency. Check external software programs for information regarding patient services, eligibility and reimbursement criteria. Check physicians licenses and monitors computer programs for information on issues with physicians licenses.

Job Description:

Collaboration

  • Responsible for the intake of new referrals and obtains from referral sources all necessary information to evaluate and triage cases. 
  • Obtain all physician orders necessary to start care. 
  • Obtain pre-authorization and authorization for third party insurance. 
  • Work with clinical staff if prior authorization is required for recertification.
  • Negotiate with external case managers for start of care issues, required documentation and payment requirements.
  • Obtain all requirements from third party payers before care is provided so as to ensure prompt and timely payment for services.
  • Coordinate services with all disciplines to ensure coordination of care. 
  • Secures from referral sources all notes, orders and progress notes and discharge planning instructions for coordination of care activity. 

 Communication

  • Communicate with all members of the interdisciplinary team and with customers to ensure the needs are met of those we serve.
  • Assist management with incoming calls, MD orders and relaying information to appropriate staff.

Nursing Assessment

  • Follow the physicians and Case Managers Plan of Care for the patient which includes physical and environmental assessment using the nursing process.       

Documentation

  • Document pertinent information regarding patients condition on appropriate forms.

Problem Solving

  • Make decisions independently regarding patient care and needs during visit using the Hospice philosophy of palliative care. 
  • Participate in on-call rotation as assigned.
  • Make independent decisions regarding appropriateness of referrals for hospice service.

Education/Performance Improvement:

  • Provide education to patient and family on palliative care issues, medication, and care of patient at end of life. 
  • Keep current on pain and symptom management. 
  • Participate in PI, chart reviews, and committees. 
  • Promotes community awareness of Hospice services through public speaking.

Personal Growth and Development

  • Is available to serve on committees to represent Hospice at SEH. 
  • Seeks certification as a Hospice nurse. 
  • Attends in-services, seminars or continuing education offerings related to palliative care to increase knowledge.
  • Performs other duties as assigned.

Education, Credentials, Licenses:

  • RN who is licensed in the state where work is being performed. 
  • Meets contact hour requirements for licensure, including all state required courses.

Specialized Knowledge: 

Basic computer knowledge.  Proven communication and organization skills.  Working knowledge of hospice regulations.

Kind and Length of Experience: 

Five years of nursing experience
