Provision of admission services to potential patients and families consistent with the goals, definitions, philosophy, and purpose of the St. Elizabeth Hospice. Provision of the initial assessment and physical assessment for all patients being admitted to St. Elizabeth Hospice. Identifies and coordinates initial medication and equipment needs and completes the initial plan of care for all admissions.Demonstrate respect, dignity, kindness, and empathy in each encounter with all patients, families, visitors, and other employees regardless of cultural background.
Job Description:
Responsibilities
- Responds to all requests for admission in a timely manner.
- Conducts hospice admission process by assessing the patients and familys physical, emotional and psycho-social needs.
- Obtains physician orders for treatment under hospice services. Coordinates initial equipment and medication procurement upon admission per order.
- Performs complete nursing assessment upon admission prior to establishing individualized plan of care.
- Utilizes nursing skills and judgment to identify and prioritize urgent needs that must be addressed within the first 24 hours of admission.
- Collaborates with interdisciplinary team member, referring physicians, DME provider, long term care facilities and others as needed.
- Develops/follows plan of care when providing home hospice visits as needed for case managers.
- Makes decisions independently and within scope of practice regarding patient care and needs during visit.
- Has the ability to adjust to the unique situations encountered in the patients home.
- Participates in the clinical on-call rotation.
- Performs assigned supervisory visits for home health aides.
Communication
- Communicates accurate, pertinent and timely referral information to Admission Coordinator, Multidisciplinary Team and, if needed, hospice leadership.
- Communicates with the multidisciplinary team any special needs identified through the nursing assessment and admission visit.
- Completes documentation to support visits and completes discharge and transfer documentation.
- Communicates patient care issues to the multidisciplinary team and, if appropriate, hospice leadership.
Documentation
- Obtains necessary consents and notice of elections for admission.
- Documents within the EMR or downtime forms pertinent information regarding patients condition.
- Completes documentation to support visits and completes discharge and transfer documentation.
- Meets timeframes required for documentation submission.
- Assures documentation is accurate, complete, specific and meets all state, federal and JCAHO regulations.
Education
- Provides information for patient/family/caregiver regarding available hospice services based on individual patient needs.
- Teaches and counsels patient/family/caregiver about hospice eligibility, various levels of hospice care, end of life issues and the shift from curative care to symptom relief and quality of life issues.
- Maintain role as a patient health advocate while the patient is on service.
- Meet all skills and competencies identified by the hospice department and healthcare system.
CQI/Peer Review
- Participates in Performance Improvement and chart reviews.
- Perform other duties as assigned.
Education, Credentials, Licenses:
- Graduate of an accredited school of nursing; licensed to practice nursing in the state where work is being performed.
- Meets contact hour requirements for licensure, including all state-required courses.
- Valid Drivers license.
Specialized Knowledge:
Demonstrates advanced assessment skills appropriate for home care environment, ability to function independently; knowledge of community resources.
Kind and Length of Experience:
2-3 years of RN experience in a hospital or care facility setting.
