St. Elizabeth Healthcare is hiring an Activities Assistant for our Skilled Nursing unit located within Edgewood Hospital.Activities Assistant - Skilled Nursing
Shift Hours: Full Time, 30 hours/week, 4 days/week, 1st shift. Occasional weekend shifts.

The Activities Assistant provides for the ongoing program of activities designed to meet the interest choice and preferences as well as to meet the interest of and support the physical, mental and psychosocial well-being of each resident, encouraging both independence and interaction in the community, as well as the physical, mental and psychosocial well-being of each resident.
Demonstrate respect, dignity, kindness and empathy in each encounter with all patients, families, visitors and other employees regardless of cultural background.
Implementation
- Responsible for carry out and documentation on the type and frequency of activity programs (1 to 1, group, self-directed).
- Adjusts 1:1 activities to meet the needs of the current residents
Care Planning and Monitoring
- Contributing to the development of the comprehensive care plan.
- Monitor resident involvement in daily activities.
- Participate in twice weekly Medicare meeting with IDT when scheduled allows.
Assessment
- Completion of activity assessment, progress notes, MDS and Care Plan within established time frame.
- Responsible for communication with IDT regarding individual resident changes in activity type and/or frequency.
Scheduling
- Coordinates activities with other disciplines to ensure maximum benefit to residents and staff support.
- Assistance with distribution of monthly activity calendar.
Innovation
- Identifies ways to enhance skills through continuing education.
- Designs person appropriate activities relevant to the specific needs, interests, culture, background, etc.
Performs other duties as assigned.
Education, Credentials, Licenses:
- High School Diploma
Specialized Knowledge:
- Awareness, knowledge and interest in gerontology. Demonstrated ability to organize and coordinate multiple programs over time. Knowledge of LTC regulatory standards governing activity programs.
Kind and Length of Experience:
- Experience in a social/recreational program involving the elderly population
