The Pharmacy Technician Med Rec, under the direct supervision of a licensed Pharmacist and in accordance with all State and Federal laws, assists with the selection, procurement, storage, preparation, distribution, record keeping and financial reconciliation of all medications dispensed to patients of St. Elizabeth Healthcare. In addition the Medication Reconciliation Technicians will review, clarify, document and verify with the patient, if possible, their Prior to Admission medications within 24 hours of admission to the hospital. The technician will assure that the prior to admission (PTA) medication list is accurate, complete and up to date. While adhering to the ICARE principles and exhibiting the AIDET principles, demonstrates respect, dignity, kindness and empathy in each encounter with all patients, families, visitors and other employees regardless of cultural background.Job Description:
Perform patient medication history:
Actively include the patient and/or the patient's family.
- Document the patient prior to admission medications accurately and completely.
- Contacts the patient's outside pharmacist or physician to resolve all discrepancies in a timely manner.
- Documents resolution of discrepancies in the medical record so that the attending physician can review and reconcile the list.
- Sets priorities in correct order in relation to the needs and circumstances at the time with reference to the patients, co-workers physicians, and the nursing unit, etc.
- Expedites medication delivery to ED areas and patient transfer medications as time allows.
- Consummate team player who epitomizes the ICARE values and expectations.
Provides superior customer service while following the AIDET principles
Answers the telephone in a timely and courteous manner, answering queries or directing the call to the appropriate person for resolution. Processes Epic messages in a timely manner.
When necessary assists the pharmacist or nurse in the ED in production of sterile preparations for patient use by aseptically compounding, labeling and storing product, in a timely and accurate manner as defined in departmental procedure.
Assists in the ongoing efforts to improve the Quality of Patient Care through development and participation in the Process Improvement Program and compliance with all policies and procedures related to Infection Control and Safety. Resulting in improved patient safety.
Participates in expiration date checking in ED and other assigned areas. Ensures that all expiration dates for medications refilled/loaded in Pyxis Med stations have been checked and stock rotated to ensure shortest date is used first.
Perform other duties as assigned.
Education, Credentials, Licenses:
- High School Graduate
- PTCB or NHA certification
- BOP Registration in state where work is being performed per state regulations.
Specialized Knowledge:
- Critical thinking and problem solving.
- Excellent verbal and written communication skills that reflect the ICARE values
- Medical terminology
- Knowledge of brand and generic medication names for the top 100 drugs as well as drug schedule and purpose.
- Knowledge of AHFS medication classes
- Understanding of drug/drug interactions and duplicate therapy analysis
Kind and Length of Experience:
- 6 months of appropriate pharmacy experience.
DESIRABLE
- Experience working with diverse populations.
- Proficient in multiple languages.
- Perform medication histories
- Prescription prior authorization submission experience
