Responsible for collecting information regarding births, stillbirths, and deaths of newborns in order to complete the birth certificate as well as collect and report statistical information required by state and federal laws.Associates are required to maintain the confidentiality of all patients both while on and off duty.

Job Description:
- Gathers information through researching medical record, Epic and patient interview in order to complete the legal birth certificate.
- Completes the legal birth certificate and submits within the specified guidelines established by state law for retention by the Bureau of Vital Statistics.
- Completes and notarizes paternity affidavits to assist unmarried parents establish paternity within the Commonwealth of Kentucky.
- Provides verification of application for social security numbers to mothers to assist them with their administrative needs. Enters baby name in Epic to assist physicians and patient needs.
- Performs other duties as assigned.
Education, Credentials, Licenses:
- High School Diploma or GED;
- Notary or ability to obtain
Specialized Knowledge:
- Typing skills of 35 wpm;
- Computer experience, Microsoft applications
Kind and Length of Experience:
- One year clerical experience;
- Proven verbal communication skills
