Job Summary:

The Advanced Practice Provider (APP) provides care and treatment to patients under the auspices of a designated physician through a collaborative practice agreement. Responsibility includes patient assessment, initiation and evaluation of diagnostic testing, documentation of physical findings, and formulation of treatment plan and prognosis based on patient’s medical condition. In accordance with established collaborative practice agreement and evidence-based guidelines, the APP obtains health histories, performs physical examinations, identifies problems, establishes goals, and formulates comprehensive plans of care for patients and teaches preventive health measures. Functions as a facilitator to coordinate clinical care, educate patients and families, and support collaboration between various disciplines and agencies, as necessary. Applies in-depth knowledge, expert skill, and clinical judgement in the critical care setting.

Additionally, the APP HVI Critical Care Team Lead oversees the Cardiac Surgery APPs including creation of 24/7/365 schedules, ensure consistent onboarding, education/mentoring to new Critical Care APPs, and collaboration with the Quality Department to ensure cardiac surgery data and documentation meets regulatory and accreditation standards.

Job Description:

Direct Patient Care Provide care to critical care/advanced heart failure patients including assessment, comprehensive evaluation, interpretation of diagnostic data, and coordination of patient plans of care as well as staff, patient, and caregiver education. Demonstrates knowledge and understanding of St. Elizabeth Healthcare’s clinical pathways (for appropriate area of practice) and applicable clinical policies and procedures. Demonstrates competence in patient assessment, prescribing of pharmaceutical agents, utilization of evidence-based practices and performance of skills in accordance with the core and non-core competencies for critical care. Initiates medical orders including, but not limited to, orders for prescribing and dispensing certain medications within the APP scope of practice as defined by applicable state law and established hospital protocols. Team Lead Responsibilities In collaboration with the Medical Director and Assistant Vice President, coordinates a diverse APP inpatient team. Participates in APP hiring, development, and job performance/annual reviews; provides ongoing coaching/constructive feedback. Develops the APP schedule and associated work hours to meet the clinical needs of the service and collaborating surgeons. Tracks number of APP shifts worked to ensure bi-annual compensation reconciliation is accurate. Ensures APP schedule is entered into approved platforms to facilitate communication of consults and after-hours patient calls. Helps to develop and implement short and long-range goals/objectives with key stakeholders. Monitors quality initiatives to meet goals and sustain results. Collaborates with surgeons and system-wide leaders in strategic planning, decision making, goal setting, coordination of patient care/services, and allocation of resources. Along with collaborating surgeons, directs and oversees development and implementation of policies and procedures utilizing current research, evidence-based clinical practice, professional clinical practice guidelines, national accreditation standards, and healthcare trends to develop evidence-based protocols governing client/patient clinical care and activities. Maintains effective system-wide communication and collaboration with various departments such as surgery, the outpatient clinic, and disciplines including physicians, patients, visitors and other stakeholders. Promotes APP engagement initiatives including provider wellness and knowledge of available resources. Encourages service excellence to provide high patient experience. Planning/Organizing/Initiating Organize and prioritize clinical responsibilities to maximize time, efficiency, and productivity. Implements and evaluates progress toward goals for prescribed plan of care based on protocols/guidelines/pathways. Provides patient care information to staff and collaborates with physicians about the patient’s progress. Participates in the quality improvement activities and community education. Communication Maintain effective verbal and written communication skills with all members of the healthcare team to promote continuity of care. Documentation in the patient’s medical record regarding all treatments and interventions is clear, concise, and completed in a timely manner. Interactions with all health care professionals are conducted in a professional manner. Keeps hospital staff/management informed of pertinent information. Provides educational sessions for facility staff as needed. General Work Practice Maintain a safe work environment, which is conducive to and complies with all state, federal, and facility and regulatory standards, policies, and procedures. Maintains a current collaborative practice agreement with applicable physician(s). Adheres to HIPAA guidelines, professional code of ethics and appropriate billing practices in executing role. Health and Safety Adhere to infection control, emergency, safety, fire and disaster plans and policies, completing required initial training and updates. Demonstrates consistent use of standard precautions and proper body mechanics. Follows OSHA guidelines. Provides a safe environment to protect patients, visitors, and facility staff. Clinical Research Participate in the development, implementation, data collection and interpretation of clinical research studies and databases in area of specialty. Utilizes and communicates current research literature to enhance care and outcomes. Reviews and updates treatment care plans for adherence to current evidence-based practice. Personal Growth and Development Evaluates own educational needs and seeks to enhance knowledge base. Participates in professional organizations associated with area of specialty. Performs other duties as assigned. Education, Credentials, Licenses (APRN): Master’s degree in nursing with National Nurse Practitioner Certification in appropriate specialty area. (Nurse Practitioner by a national certifying organization recognized by the Board of Nursing-Acute Care or Adult, Gerontology-Acute Care in the state where work is being performed) If eligible, must have RN Compact (multi-state) License in their primary state of residence. Graduate of an accredited school of nursing Licensed as an RN in the state where work is being performed. Requires Certificate of Authority and Standard Care Arrangement with collaborating physician to practice as a nurse practitioner. Must have medical staff privileges as an allied health professional. Advanced Cardiac Life Support (ACLS) Education, Credentials, Licenses (PA-C): Graduate of an accredited Physician Assistant Program; meets the requirement for certification/re-certification by the National Commission on Certification of Physician Assistants (NCCPA) in appropriate specialty area. Licensure as a Physician Assistant in the state where work is being performed; completion of CME requirements. Requires Certificate of Authority and Standard Care Arrangement with collaborating physician to practice as a physician assistant. St. Elizabeth Medical Staff Membership & Privileges. DEA Certificate with prescribing/dispensing privileges in the state where work is being performed. Advanced Cardiac Life Support (ACLS) Specialized Knowledge: Demonstrated skills and knowledge as an Advanced Practice Provider in area of expertise. Kind and Length of Experience: For APRN, 5 years of experience as a registered nurse. For APRN, work as a registered nurse in a hospital critical care setting For PA-C, at least two years of experience in a hospital critical care setting.
